SINGAPORE: Last month, the Singapore Government announced that it would guarantee all bank deposits, backed by some S$150 billion from its reserves.

It marks the first time the elected president is exercising his custodial powers to approve a potential draw on reserves.

Speaking to the media for the first time on the issue, President S R Nathan said he gave the go-ahead as a contingency plan, but the need to draw on the reserves has not come about yet.

He said: "If it does come, I've got a Council of Presidential Advisors, all of whom are knowledgeable, who know the economy (and) finance… who will have to see through and see justification for the draw. And that's the procedure we will follow.”

'DBS would never knowingly do anything that will hurt customers,' Mr Stanley said, speaking at the release of DBS' third-quarter results yesterday.

Mr Stanley, an ex-Citibanker who took over the reins at DBS only six months ago, said the episode has been 'extremely painful' for DBS.

'I've seen our relationship managers - grown men and women - in tears as they broke the bad news to their clients.'

In Singapore and Hong Kong, more than 4,700 customers invested a total of $360 million in Lehman-related notes. In Singapore, 1,400 customers invested a total of $103 million in DBS High Notes 5 alone.

'We've been very concerned for our customers, so from day one, we spared no effort in being open and transparent with them,' added Mr Stanley.

Mr Stanley said that customers both here and abroad regard DBS as a 'steward of their hard-earned savings and wealth'.

'With this privilege comes responsibility and we take this responsibility very seriously,' he added.

Kidney donors can be reimbursed, says ethics panel
But amount should not be so high as to induce them to donate

THE Health Minister's proposal to reimburse those who donate a kidney has cleared a national committee on medical ethics, paving the way for a change in legislation to be put before Parliament early next year.

The 18-member committee, led by Dr Lee Suan Yew, had no issue with reimbursing donors so long as the sum is not so large as to become 'an undue inducement, nor amounting to organ trading'.

It supported the principle of 'comprehensive reimbursement', and spelt out what donors can expect to get.

This includes being paid for expenses relating to the donation, such as transport and medical costs, and for loss of earnings that come as a result. Also, the donor should be covered for follow-up medical costs and higher insurance premiums as a result of losing a kidney.

To ensure the donor can be looked after properly following the operation, it suggested that the reimbursement scheme start with donors who are Singapore citizens and permanent residents.

'Comprehensive reimbursement of verifiable and reasonable expenses of kidney donation does not constitute payment for that organ, and may be viewed as part of the legitimate costs of treating the recipient,' it said.

The committee, set up in 1994, was asked to look into the ethical dimensions of the proposal, mooted by Health Minister Khaw Boon Wan in the wake of a high-profile organ-trading court case in July.

Retail magnate Tang Wee Sung was convicted of trying to buy a kidney from an Indonesian, who was to have been paid $23,700.

The case aroused much debate, as well as renewed focus on the reluctance of people to give up one kidney to save a life.

Dr Lee said the committee spent close to three hours debating the subject because 'everyone was so excited and keen to express their opinion'.

The decision, he said, was unanimous.

There are about 600 patients waiting for a kidney, despite changes to the Human Organ Transplant Act to increase the pool of cadaveric organs, such as including Muslims who die.

Mr Khaw has said the reimbursement will be at least a five-figure sum, possibly even six-figure.

Mechanics such as who should disburse the sums and how have yet to be firmed up, but the National Kidney Foundation (NKF) is likely to take on the role.

NKF chairman Gerard Ee said he hopes the scheme will encourage more family members to come forward 'by removing the reasons for not wanting to donate'.

The reimbursement could take the form of NKF paying for hospital treatments or higher insurance premiums. Aside from making up for loss of income, no cash should change hands, he said.

He also wants a cap on the number of years for which medical treatment is paid for. This is because medical problems caused by giving up a kidney are unlikely to occur decades after.

In its statement yesterday, the ethics committee said it looked at guidelines from the World Health Organisation and the Declaration of Istanbul on Organ Trading and Transplant Tourism promulgated in May this year to draw its conclusions.

But it also stressed the importance of greater efforts to prevent kidney failure, and increasing the cadaveric organ donations.

Said Dr Lee: 'Transplant is just a solution to a problem. The real answer is to prevent the need for transplants.'

Madam Halimah Yacob, who chairs the Government Parliamentary Committee on Health, said carrying out the proposal will be 'the real challenge', since the line between organ trading and reimbursement is 'tenuous'.

She also referred to the committee's recommendation that donors be drawn from the ranks of citizens and permanent residents to facilitate post-operative care. The ministry, she suggested, could tie up with hospitals overseas to ensure that foreign donors are cared for.

But Dr Lee was adamant that including foreigners could lead to 'backdoor trading'. This, he said, would damage Singapore's reputation as a medical hub.

More cases, more places
170 locally transmitted cases since August; Kaki Bukit and Woodlands among areas hit

THE mosquito-borne disease chikungunya is continuing its march here: More people have become infected in more locations.

From August up to now, 170 cases have been contracted here, as opposed to having been brought in from overseas.

After a spike to 23 cases a week in early August, the number of local transmissions has remained high - and this comes after only 23 local cases in the first seven months of the year.

The infection also appears to have spread widely, with people getting bitten by carrier Aedes mosquitoes in places as diverse as Toa Payoh, Woodlands and Kaki Bukit.

A Health Ministry spokesman said the spread of the disease to many locations showed that Singapore remained highly susceptible to it. She put it down to the 'significant presence' of the Aedes albopictus mosquito.

Of the approximately 350 people who have come down with chikungunya so far this year, the majority caught it here; most of those who caught it abroad did so in Johor. The pattern is the same for the last three weeks: Of the total of 62 cases, 47 or three-quarters were caught locally.

The National Environment Agency has mounted a campaign this year to wipe out carrier mosquitoes and their breeding sites.

The Health Ministry said, however, that although local transmissions are now outnumbering foreign ones, the disease cannot be considered endemic as there are still imported cases.

Several more months of monitoring are needed before such a call is made, the spokesman added. If a disease is labelled endemic, it means it cannot be eradicated.

By early next year, the ministry will make chikungunya a notifiable disease, like malaria and Aids. This means that if a doctor comes across a chikungunya case and does not report it to the ministry, he could be jailed for up to six months and/or fined up to $10,000.

Doctors have, however, been told to report cases since December 2006, when the number of infections in the region started rising.

Chikungunya causes intense joint pain that could last for months.

Victims may also have chills, vomiting, rashes and fever for three to 10 days. Symptoms could take a week or more to appear after the person has been infected.

As a precaution, the blood bank has urged donors to wait two weeks before donating blood if they have been to places where chikungunya is endemic. This is to prevent the disease from spreading through infected blood.

This precaution will be futile if the infection becomes endemic here.

If that comes to pass, the effects of chikungunya could be as bad as dengue, which infects thousands of people here each year.

Dengue, which has symptoms similar to chikungunya, is also spread by mosquitoes, but of a different breed - the Aedes aegypti.

Last week, 135 people were diagnosed with dengue, bringing the total for this year to 5,428.

Nation's interests must come before race: PM

Singapore's leaders must put national interests first before those of their particular racial community, Prime Minister Lee Hsien Loong made it clear yesterday.

That is why the country will not head down the road of electing people to Parliament to represent the interests of their race, as is the case in Malaysia and New Zealand.

In response to a question on whether Malay Members of Parliament have difficulty balancing their community's interest with the national interest, Mr Lee explained what he expected of MPs here.

'If he is a Chinese, I want to ask him if he is a Singaporean who happens to be of Chinese descent or is he Chinese first, who happens to be living in Singapore,' he said.

Singapore is unlike Malaysia as it does not have 'a coalition government where there is a Chinese party and a Malay party and an Indian party, and then we try to push and see who can push harder'.

Neither does it intend to move towards a system of proportional representation as practised in New Zealand, where the small number of MPs voted in to represent Maoris can exact a price each time the government seeks their support.

'But for Singapore, I think it would be the wrong thing to do. In Singapore, you come in, you have to think about the interests of Singapore,' Mr Lee said.

On the difficulty of finding Malay political talent, he said it was tough to find such talent across all the races.

Although the pool of Malay talent is bigger than what it used to be, the proportion of Malay professionals - such as doctors, lawyers and engineers - is still lower than the national average, he said.

Mr Lee also called for the grassroots leaders' help in talent-spotting, asking: 'Do you know anyone to introduce to me?'

'I have quite reluctantly come to a conclusion that my view is an idealistic view and that there are certain political realities I have to face.'

In Singapore, if a good Indian candidate is pitted against an articulate Chinese candidate in a single constituency where 75 per cent of the people are Chinese, 'I think it will be very difficult for the Indian or Malay candidate to win, especially if it's the first time'.

AVA allows some China-made milk products to be back on sale

SINGAPORE: The Agri-Food and Veterinary Authority (AVA) has given the go-ahead for some China milk products to be back on sale with immediate effect. These include butter, ghee, non-dairy creamer and cereals.

Over the next three weeks, AVA will continue testing other China-made products and those deemed safe will return to the stores.

The first batch of approved products that passed two rounds of tests and were deemed low-risk were back on supermarket shelves.

Also falling under the same category were products made in China with milk from other countries.

But local importers will have to give documentary proof of the milk's origin to AVA and the products must undergo tests before they can be brought in.

AVA has separated the products for testing into risk categories of high, medium and low. That was determined by factors such as the level of milk content and frequency of consumption.

For instance, some non-dairy creamers could still contain about two per cent of milk, as they contain a form of milk protein called casein.

High-risk products include milk and ice cream. Medium-risk products include biscuits, cake, bread, chocolate, crackers, candies and other confectionery products.

Dr Chua Sin Bin, CEO, AVA, said: "When we lift the ban on import, we will require the product to be fresh products because this is a man-made problem. The moment they are able to stop, the new products will be safe."

The agency said a team will head to China soon for a field inspection visit for about a week, including farms and milk collection centres.

AVA will consider lifting the current ban after they have a clear picture of the improvements the Chinese have made to their food safety system. - CNA/vm

SINGAPORE : Health Minister Khaw Boon Wan said construction plans for the new Jurong General Hospital have been brought forward to take advantage of softening construction costs.

In July, the government has said it is deferring another S$1.7 billion of public sector construction projects to ease pressure on the then red-hot construction sector.

More hospital bed spaces are coming on-stream and drugs could get cheaper, thanks to the economic crisis.

Private healthcare operator ParkwayHealth is building a 350-room hospital which will be ready by 2011.

The hospital will specialise in four main clinical areas - musculoskeletal, heart and vascular, oncology and general surgery.

ParkwayHealth has seen a 5-7 per cent drop in medical tourists in recent months due to the financial crisis. It expects the fall to taper off at 10 per cent.

But it is confident that in the long run, patients from the Middle East, Russia, Ukraine, Kazakhstan and the Asian region will continue to seek treatment in Singapore.

The national aim is to treat one million foreign patients a year by 2012.

ParkwayHealth said the drop in medical tourists from Indonesia in recent months has been most pronounced. But more patients are coming from Vietnam and Bangladesh.

"This is about the best time to build a hospital whereby we have more time, and our hands are not so tied with operational problems, so we can pay more attention to the construction," said Dr Lim Cheok Peng, managing director of Parkway Holdings Ltd.

Construction costs in Singapore have been falling due to a slowing economy, and this is good news for ParkwayHealth's new hospital at Novena which is estimated to cost between S$300 million and S$500 million.

The health minister also said he will bring forward the construction of the public hospital in Jurong to take advantage of falling construction costs.

Jurong General Hospital was originally slated for completion in 2015, but it's likely to be earlier now.

Minister Khaw added that another key reason is that hospitals are operating near capacity.

"We have been enjoying a very steady growth - almost 20 per cent per annum compound rate of foreign patients. But last year, compared to 2006, it was quite flat. I don't think it is because demand has shrunk or we are losing competitiveness, but capacity is constrained. When I visit friends who go to private hospitals, I look at the ward.... (it's) just like public hospitals," said Khaw.

To address the shortage of hospital beds, Singapore HealthPartners, a partnership formed by local doctors, is also building a "mediplex", which consists of a hospital, hotel and specialist medical centre, in Little India.

But it's not just a shortage of beds, there's also a shortage of doctors.

Over 600 doctors graduated in Singapore in the last three years, while over 1,000 foreign medical graduates were recruited over the same time.

And there is still a need of more doctors to service new citizens and foreign patients. Recruitment is not expected to slow down with the recession, and no pay cuts are expected for public sector nurses and doctors.

Mr Khaw said it is important to pay them the market rate, otherwise the industry may lose 300 specialists by the time the new hospital opens.

Another advantage that the slowing economy is the possibility of cheaper drugs for patients, as the steep increase in drug prices in the last two years were partly due to high oil prices. - CNA /ls

Shortage of construction workers and materials causing costs to shoot up by as much as 50%

SOME building projects in Singapore are facing cost blowouts of 30 to 50 per cent above their original budget as higher construction costs bite hard.

The continuing shortage of construction workers and building materials has left project bosses with little choice but to pay upfront for the far higher costs or abandon the project.

Anecdotal evidence suggests that building budget blowouts are far bigger than official figures indicate.

One project that has been hit by higher building costs is the Khoo Teck Puat Hospital in Yishun.

The chief executive of Alexandra Hospital, Mr Liak Teng Lit, who is overseeing the construction project, said some parts of the project face cost hikes of 30 to 40 per cent. The original total cost of the project was $400 million.

Another project that has been hit is Safra's Jurong clubhouse. Costs for the project have shot up 30 per cent since its groundbreaking ceremony in February last year. Its original budget: $30 million.

And earlier this month, the Health Ministry announced that it would provide Ren Ci charity with additional funding of 'up to $9.3 million' for its new hospital at Irrawaddy Road.

This was to help cover the hike in construction costs for the medical centre - now set to cost up to $42.4 million.

Singapore Zoo assures visitors that white tiger exhibit is safe

SINGAPORE: A day after a cleaner was mauled to death at the Singapore Zoo's white tiger enclosure, officials assured the public that the exhibit is "very safe".

It held a news conference on Friday to clear any doubts even as it opened its latest S$12 million attraction.

Business was as usual at the Singapore. Visitors streamed in to view the latest attraction called the Rainforest Kidzworld which opened in time for the school holidays.

Gracing the launch was executive director and CEO of Temasek Holdings, Ms Ho Ching.

She said: "The zoo has deepened its own knowledge and expertise through the years. I have full respect for the team at the WildLife Reserves for their hard work, passion and constant care. It is amazing how their dedication and imagination have created a natural and safe wildlife habitat only half an hour away from the city lights."

The chairman of Wildlife Reserves, which owns the zoo, took the opportunity to speak on the incident.

Claire Chiang, non-executive chairman, Wildlife Reserves, said: "Let me take a few minutes to address the unfortunate incident at the white tiger exhibit yesterday. Our sympathies and condolences go to Mr Nordin Bin Montong's family. I would like to reassure all visitors that the white tiger exhibit is very safe and is as safe as any part of the zoo.

"The safety measures we have implemented exceed the standards recommended by the association of zoos and aquariums (AZA). AZA is the internationally recognised organisation that accredits only institutions that have achieved rigorous standards for animal care, education, wildlife conservation and science."

Visitors too, didn't seem too concerned about safety.

Speeding elephant
MR TAN JUAN YEW: 'My family and I visited the Singapore Zoo last Saturday. As we rested in a shelter, we saw an elephant speed past. A zoo employee riding a bicycle beside it rang his bell frantically to visitors to flee from the elephant's path. We broke into a cold sweat when we saw a pram in the middle of its path. Fortunately, the father quickly pushed it aside just before the elephant ran past. Later, we found out that the pram was empty as the baby was with its mother. Still, it was dangerous, especially for children and the elderly who may not be able to react in time.'


Aaron Tan, a visitor, said: "The enclosure is actually quite safe as long as you don't go close into the enclosure itself. So I can still bring my kid here."

At a news conference later, zoo officials addressed other concerns raised, including the five-minute response time it took the sharpshooters to reach the scene.

Biswajit Guha, assistant director, Singapore Zoo, said: "The five-minute situation is essentially sufficient to go to a hotspot in the case of an animal escape. This was an extraordinary situation whereby someone had actually jumped in.

“Even if it had been a fall and the person was trying very hard to avoid the tigers and had gone to the deep end, we are quite confident he would have been able to move in, in time."

He added that firing tranquilisers would not have saved the cleaner as it could have further provoked the animals.

He added: "Tranquilisers don't work instantaneously. So it will take about five minutes before having an impact on the animal. And to have such a sharp impact coming into contact with an animal, it could provoke the animal more and there might be a more drastic reaction."

Meanwhile, the Singapore Zoo said it will not be stationing armed officers at the enclosures which are deemed dangerous. But it will definitely be increasing its patrols conducted by the zoo's keepers and operational staff.

More "die-logues" expected as govt considers changes to AMD

SINGAPORE: The government is considering amending the law to encourage more Singaporeans to sign the Advance Medical Directive (AMD).

The AMD is a legal document which allows an individual to tell doctors that he does not want his life to be artificially prolonged when he is terminally ill and unconscious.

Health Minister Khaw Boon Wan told Parliament on Monday that changes to the Advance Medical Directive Act may include doing away with the requirement to have a doctor witness and allowing doctors and patients to actively discuss AMDs.

Only 10,100 Singaporeans have made a living will since the AMD came into effect 11 years ago.

Three-quarters did so over the past four years. Six have been put to effect while 19 AMDs have been revoked over the years.

One factor for the low take-up rate is that the AMD Act does not allow doctors and nurses involved in the care of a patient to ask whether he has made or intends to make an AMD.

There are severe penalties including a fine and a jail term for those who do so.

Mr Khaw said: "I gather that these restrictions were put in as there were concerns by Singaporeans that a doctor may be less forthcoming in his treatment if he knows that his patient has already signed an AMD. Unfortunately, such fear, though groundless, remains among some Singaporeans.

"I am confident that our doctors are ethical and will do what is necessary for their patients, regardless of their AMD status. The restrictions have inadvertently become counterproductive to the intent of the AMD by making discussions on AMD taboo. This is a major reason for the lack of awareness and apathy among the public and healthcare professions on the AMD.”

Mr Khaw said he's keen to remove this restriction as it has made AMD a taboo subject between doctors and patients.

The Health Minister added that his ministry will ramp up its outreach efforts and encourage more “die-logues” to help Singaporeans better understand the difference between AMD and euthanasia or doctor-assisted suicide.

Mr Khaw said that to make the meaning of AMD even clearer, he is considering giving it a new name.

The Health Minister also stressed that the AMD is not euthanasia.

Mr Khaw said AMD is about letting nature take its course for the terminally ill in the last stages of their life.
