the new report that his life jacket was not inflated. i used that b4 during BX water evacation drill. trust me, once inflated, confirm rise to the surface. the only thing not sure was the life jacket malfunction or now. maybe the gas cylinder broke so cannot inflate the life jacket. knowing SAF, maybe the gas cylinder attached to the life jacket expired.

Only one possibility why it did not inflate, the fella must be unconsicous when he fell

or he don't want to sign 1206 for a spent gas cylinder and didn't pull the tab to inflate and think he can out swim the currents.

Very unlikely to be equipment failure.

If the work culture remains the same for the past 30 years for army coxswains; I believe this is the only two possibilities.

All designated safety boats usually have a boat commander and a boat 2IC in charge of a detachment other than the commander from the attachment unit.
What is surprising is no one notice the fella fell which is the questionable part. In a convoy its easy to see everything.

I forsee a lot of heads will roll after this episode but it will never be reveal to the public.

pls lah, i can accept the unconsicous part but the part where the lcp didnt pull the tag because of 1206 is unacceptable. to use the equipment no need to sign 1206 lah, only need to sign 1206 because equipment damage or lost due to neligence. at most fill a form to expense it off.

as for the convoy part, there had be many accidents happened yet none of the ppl in a convoy noticing. my platoonmate on attachment to brigade for wallaby drove the M113 off a bridge but nobody notice it. not even the safety rover at the rear of the convoy. it only when return to base camp that people finally notice one M113 missing. he and his VC was at the dry riverbed for hours before landrover found them. both were hurt badly and downgraded due to the injuries.

the lcp was in a 4 boat convoy and looking at the terrain, it a single file convoy with the lcp being the last boat and ppl tend to look to the front so there a chance the lcp felled into the river without the boatman in front noticing. i dunno how noisy the outboard is but there a chance his cries for help could not be heard over the engine noise.

as for equipment failure, maybe u lucky, neber experience equipment malfunction in your NSF & 13yrs cycle. i sure there are many ppl here who will disagree from their own SAF experience.

Please specify who's head will roll? For sure the HEN will still be defence ministar. The same with his deputies. I think they will only make the junior commanders as scapegoats by demoting them
