SINGAPORE: An independent Committee of Inquiry (COI) will be convened to investigate a Singapore Armed Forces (SAF) training incident in New Zealand, which led to the death of actor Aloysius Pang.
More information about the circumstances of what happened is expected to be revealed at a Ministry of Defence media conference on Thursday afternoon (Jan 24).

This is the fourth time in 18 months where an SAF soldier has died while in training.
Jan 19, 2019: Corporal First Class (National Service) Pang, 28, was carrying out repair work inside a Singapore Self-Propelled Howitzer - an artillery gun mounted on an armoured chassis - with two other SAF personnel when an accident happened, seriously injuring him.
He had been taking part in Exercise Thunder Warrior at the Waiouru Training Area as part of his reservist commitments and was injured when the gun barrel was lowered.
Pang had to be put on artificial life support for his lungs, kidneys and heart, and died at 8.45pm (Singapore time) on Wednesday at Waikato Hospital in Hamilton.

Full-time national serviceman (NSF) Liu Kai died after he was involved in an accident at Jalan Murai training area on Nov 3.
Corporal First Class (CFC) Liu Kai was operating a Land Rover as part of a field training exercise when a Bionix vehicle reversed into his vehicle, partially mounting it. The 22-year-old died from his injuries and an army-wide safety timeout was called, including overseas training exercises.
Defence Minister Ng Eng Hen told Parliament on Nov 19: "There are obviously a number of questions that need to be answered in determining the cause of this incident.
"They include: Were safety protocols followed by the crew of the Land Rover and the Bionix vehicle during this phase of the exercise? Were the vehicle commander, driver and crew of the Bionix aware of the Land Rover behind them and did they conduct their reversal safely?"
A COI and police investigations will fully examine several questions into the death of CFC Liu, he said.

Full-time national serviceman (NSF) Dave Lee died on Apr 30, nearly two weeks after "displaying signs of heat injury" in Bedok Camp following the completion of an 8km fast march.
The COI investigating the death of the full-time national serviceman found that inadequate on-site casualty management and delayed evacuation were "likely reasons" for him succumbing to heat stroke.
Citing preliminary findings, Defence Minister Ng said in a ministerial statement in Parliament on Aug 6 that there were several calls for CFC Lee to be evacuated, but these calls were either "not heard or not heeded".
"The delay in evacuation resulted as the persons attending to CFC Lee mistook his signs and symptoms as due to physical exhaustion," Dr Ng said.
The COI also found breaches of training safety and discipline regulations on Apr 17, the day before the heat stroke occurred.
Dr Ng told Parliament that if no criminal charges are filed against those responsible for lapses leading to the NSF's death, the Ministry of Defence (MINDEF) would prosecute them in a military court.

3rd Sergeant (3SG) Gavin Chan, 21, died after he was involved in a vehicular accident at the Shoalwater Bay Training Area in Queensland, Australia in September 2017.
According to MINDEF, 3SG Chan, a Vehicle Commander from 41st Battalion Singapore Armoured Regiment, was travelling in a Bionix Infantry Fighting Vehicle as part of an overseas training exercise.
3SG Chan had been giving instructions to the driver of the Bionix vehicle when it got stuck on a large boulder whilst reversing. The vehicle suddenly "dropped down over the rock rolling sideways before coming to rest further down the slope”, a coroner's report said.
According to Queensland police investigations, he was “ejected from the vehicle and suffered significant injuries”.
There were some safety lapses identified by a COI, but it did not find any “negligence, foul play or misconduct”, Defence Minister Ng said. However, the COI recommended the SAF review its training safety regulations on the position of the vehicle commander for armoured fighting vehicles.

RIP. I've known many guys who died during full-time NS or reservist whom most never read about on the newspapers (or the news took up merely a small column or paragraph tucked away in the corner). Many of them died in a far more horrible way.

Because this guy was a mini 'celebrity' rather than some local peasant's son, his death has magically transformed into a 'national tragedy'. It's still a tragedy nonetheless, but the hypocrisy is clear for all to see.
Good that he got a little fame.

SINGAPORE: The death of actor and national serviceman (NSman) Aloysius Pang hit close to home for Workers’ Party (WP) chief Pritam Singh, who is currently on reservist duty.
In a Facebook post on Thursday (Jan 24), Mr Singh said he has been back in camp since Monday.

“Like many Singaporeans, I have been both anxious and worried about one of our NSmen who suffered a training accident in New Zealand, particularly after his medical condition deteriorated,” he wrote.
“Having been in uniform for the last four days, the news of his death last night is more acute, with the loss of a fellow NSman hitting closer to home.”

Pang, 28, died on Wednesday from injuries he sustained during a Singapore Armed Forces (SAF) training exercise in New Zealand.

He had been carrying out repair work inside a howitzer - an artillery gun mounted on an armoured chassis – and was injured when the gun barrel was lowered, the Ministry of Defence (MINDEF) said.

Mr Singh, an NSman who serves as a combat engineer and holds the rank of major, said National Service training has evolved to become more focused.
“The food quality has increased by leaps and bounds. We have become more conscious of safety and balancing it with realistic training. The quality of our personal equipment, uniforms etc. is better than before,” the WP secretary-general wrote.
However, one thing remains constant, he said. “That sense of camaraderie - regardless of rank, race or religion when we swap our civvies for uniform, even if it is only for a few weeks a year.
“Like all NSmen and NSF personnel who have passed on while on duty, we pray for strength for their families and loved ones through difficult times. And we stand with them.”


MINDEF said an independent Committee of Inquiry (COI) will be convened to investigate the circumstances leading to Pang’s accident.
Preliminary findings will be shared in a press conference chaired by the Chief of Defence Force on Thursday afternoon.

Corporal First Class (CFC) Pang and two teammates were trying to diagnose a suspected fault in the gun, when he was caught between the end of the gun barrel and the cabin and sustained “crush injuries”, he told reporters at a media conference.

CFC Pang, 28, was part of a maintenance team called in to carry out repair work inside the self-propelled howitzer — an artillery gun mounted on an armoured chassis.

His teammates, a detachment commander and a fellow technician were with him in the cabin of the howitzer, but neither was injured.


The end of the gun barrel juts into the cabin when lowered, reducing room within the vehicle.

Technicians are trained to move out of the way and find a safe space when the barrel is lowered, Major-General Goh said.

“It is typically sufficient for our artillery operators as well as our technicians to be able to operate within this cabin,” he added.

It takes an average of nine to 10 seconds for the gun barrel to be lowered to its standby position, said Colonel Terry Tan, commander of the Singapore Armed Forces (SAF) Combat Service Support Command.

CFC Pang, an operationally ready armament technician, was believed to be standing when the barrel struck him.

Major-General Goh was asked for more details on what happened, such as whether Pang was facing the gun barrel when he was struck and whether he failed to move at all or moved too slowly, but he declined to go into the specifics, saying these details will be investigated by an independent Committee of Inquiry.

As part of standard operating procedures in lowering the gun barrel, a command is given to lower the barrel along with a verbal warning, and the commander also inspects the cabin visually before pushing a button, said Colonel Tan.

Major-General Goh said the SAF is not aware of any obstructions that could have impeded CFC Pang’s movement.

CFC Pang died on Wednesday night, four days after the accident left him seriously injured in his chest and abdomen.

CFC Pang, who had been serving in the 268th Battalion Singapore Artillery, was treated at the Waikato Hospital, a regional trauma centre in Hamilton, New Zealand about 150km from the city of Auckland.

The accident occurred during Exercise Thunder Warrior, an artillery live-firing exercise, around 2pm on Saturday at the Waiouru Training Area.

Wat a load or croc, if the gun is being repaired, there should be a safety switch etc that prevents the barrel from moving in the 1st place. If being repaired n the gun can still move is like connecting power lines without switching off the power
