In his Facebook post, Dr Thambya wrote that “Ministry of Health (MOH) data shows that the bed occupancy is consistently at or above maximum levels for patient safety”, with bed occupancy rates hitting as high as 95 per cent in Tan Tock Seng hospital between April 14 and April 16. According to the National Institute for Health and Care Excellence guidelines published in 2017, occupancy rates should not exceed 90 per cent in order to facilitate optimal patient flow.

“While much of this is due to overpopulation from the misguided immigration policies, a large part of this is caused by the overly complex healthcare financing system,” Dr Tambya wrote, pointing out the need for citizens to buy their own healthcare insurance plans despite the existence of national health insurance scheme MediShield Life.

Dr Tambyah noted the increasing inequality in Singapore, cautioning that Singapore is “approaching the level of the United States which probably has the most dysfunctional system in the high-income world”.

Outgoing Health Minister Mr Gan Kim Yong revealed in Parliament that there is a striking five-and-a-half-year gap in life expectancy between Singaporeans aged up to 25 with secondary education and their peers with post-secondary education.

It was revealed that “men with below secondary education level were 1.7 times more likely to have diabetes, 1.2 times more likely to have hypertension and 1.2 times more likely to have high cholesterol, as compared to those with post-secondary education. For women with below secondary education level, the chances were 1.5 times for diabetes, 1.7 times for hypertension, and 1.4 times for high cholesterol compared to those with post-secondary education.” according to the reply by the MOH.

Dr Tambyah urged these issues be addressed, or the gaps in our society will only widen.

After Israel’s national campaign vaccinated more than three-fourths of adults 70 years or older against SARS-CoV-2, the need for mechanical ventilation among patients with COVID-19 in that age group declined dramatically.

The country’s vaccination campaign began in December 2020 by prioritizing adults 60 years or older, health care workers, and those whose chronic medical conditions increased their risk of severe COVID-19. By February, 84% of adults 70 years or older had received 2 doses of the Pfizer-BioNTech vaccine. To determine whether the vaccinations prevented severe disease, researchers calculated the ratio of patients aged 70 years or older to those aged 50 years or younger with COVID-19 who required mechanical ventilation. The younger group was chosen as a comparator because its 2-dose vaccination coverage was only about 10% as of February.
From October to December 2020, before the vaccination campaign began, a daily median of 84 patients aged 70 years or older required mechanical ventilation compared with 15 patients aged 50 years or younger—a ratio of about 6 to 1. By February, that ratio fell by two-thirds to about 2 to 1. The authors noted that their analysis, along with preliminary evidence posted on a preprint server, provide real-world evidence that the vaccines effectively protect against severe disease.
“The findings from this study provide preliminary but important evidence of the effectiveness of vaccines in preventing severe cases of COVID-19 at the national level in Israel,” the authors wrote. “Receipt of COVID-19 vaccines by eligible persons can help limit spread of disease and potentially reduce the occurrence of severe disease.”

Primary healthcare is like Polyclinic and Pte GP clinics.

They all also are happy with busy business.

Even A&E.

All the departments have to be accountable as cost centres. Profits. Loss.

Polyclinic could hire more drs. A&E also. Drs sit around wait for business to come. Supply and demand.

But in reality no one will do that lah. Supply=demand. Just nice.

In fact I think they prefer that the demand is higher than supply. Then squeeze out more efficiency.

Many years ago I went to TTSH to learn value stream analysis course. Polyclinic sent me. Modeled on Toyota Production System. Analyse the step by step what happens to a patient coming into hospital or clinic or department. Then ask of all these steps which ones patient will not be willing to pay for. Can we eliminate it? Simplify it? Reduce waste. More efficient.
 
The reality with healthcare is that it is getting too expensive.

The drug costs and imaging costs and overall admin costs continue to increase.

Clinical staff wise is ok the costs have only matched inflation. Drs and nurses will still join the profession even if the pay is low. It is a privilege to serve.

I think perhaps the government should slash the payroll for drs and nurses by 25%. Use that money to offset the rise in costs in all other areas.

Dont worry about the drs and nurses they will get on just fine. They wont know what else to do. Lol!

It is not like Drs and nurses can choose to do something else or switch careers. Sure they can go private but cutting the govt salaries will also benefit the pte sector as there will be more supply of drs and nurses. Overall cost of healthcare will drop.

Drs and nurses are regulated. Meaning they have to provide standard of care. Having lower pay doesnt mean they can neglect that.

Eventually I envision drs and nursing to be a close to minimum wage position and the people working in those roles will be doing it more because they wish to serve than for money. It will make for a better healthcare system.
 
Nobody want to be a health minister during this period not in Singapore not any country. But someone has to do it, someone who has the guts to take up this mountain of challenge. Ong Yee Khun is such a man who has the balls, the courage, the gusto to pick up this job that no one want to do. He is the Man of the Hour picking up this laborious load on his shoulder knowing very well he may even die on the job. Unlike many who stood at the side only know how to kpkb like this Dr Tambyah.
