i was in sg recently to investigate the cause of sinkies' swarming of clinics and hospitals. not surprisingly, i found sinkies have becum hypochondriacs much like americans hooked on getting all sorts of medication from western-style health care to tcm, for instant relief. out of several patients i surveyed, close to half of the medication prescribed are useless and not addressing the issue which is mostly mental and based on paranoia. the other half are placebos ranging from anti-depressants to anti-allergens. there are even patients who have been taking wafarin, a blood thinning medication, for the last 2 years! no wonder they are fainting and dropping in the streets! sinkies lack good medical dialogues with their primary physicians due to language issues, plus the fact that doctors perceive them as sheep not knowing and understanding medication pitfalls, side-effects and limitations. doctors have a problem convincing sinkies to chill off and stop seeing them for mundane consultations. because sinkies want instant cure based on medication, doctors have little choice but to prescribe placebos. tons of them.

i was flabbergasted to find out that sinkie doctors in clinics are prescribing antibiotics to patients who have been infected with flu, which is viral, not bacterial. these are sinkie doctors who graduated at nus. i explained to them that vaccines combat viruses while antibiotics combat bacteria, and bacteria like microbes can still survive in harsh environments while viruses die under ultraviolet light, yet they kept pushing antibiotics to patients despite the viral ailment. this confirms your astute observation.

The reason was revealed by one of my doctors friends. He said that if he refused to prescribe antibiotics, the patient would label him as "useless" and simply look for another doctor that ceded to their demands.

While doctors are considered to be "professionals", any practice still has to be run as a business first and foremost and if you are left with no patients as a result of taking an ethical stand against the overprescription of antibiotics, you might as well close your clinic and find some other way of earning a living.

Doctors in Singapore either have to dish out antibiotics for everything or risk going under.

The other demand of sinkie patients was they all want a "jab" to quicken the healing process regardless of what sort of ailment they have. Saline solution is the cheapest placebo with no risk of side effects so that's what most patients get. The pay good money for it too and feel "much better" after a mainlining a dose of NaCl dissolved in distilled water.

then patient education is the way to go and promoting the use of pharmacies. but i'm sure many private clinics would go out of business if we went that way. vested interests are getting in the way of reform. there is also the side issue of needing MCs when we're sick, so that rules out pharmacies.

as for the elderly being abandoned, this is a social issue that has not been addressed. obviously families are facing financial difficulties in paying for the care of their parents. there is a growing need for more help in this area and calls for more and not less spending on healthcare.
