Very interesting revelation but bring out more doubts.

Firstly, who maintain your own sump pump now? It is usually sub out to contractors who use ft as it is very dirty and dead rats are commonly found there.

Secondly why non maintenance only for 1 year or longer?

Many would not know that the basement car park in your condo, commercial bldg etc may have a holding tank but there is definitely a sump pit. Many bldg manager also never maintain the sump pump and pit as water flowing into the basement is usually not great enough to cause flooding even when the pumps fail. However, it will cause 'ponding' which will be noticed by user of the car park and reported immediately. Then, do the managing agent realized that the existence of the sump pit.

The same situation may have happened. Nobody care about the holding tank as there was no problem for years. When management and staff change, the new group may not even know the existence of the holding tank. Therefore, were the maintenance records created only after the incident?

The shit started when the Fake Phaik fired all the experienced and long time maintenance personnel who were majority sinkies for cheaper FTs.

The name of IE Singapore Chairman, Seah Moon Ming, has been found inside the recent Panama Papers, released by the International Consortium of Investigative Journalism (ICIJ) (https://offshoreleaks.icij.org/nodes/117997):

The papers reveal that Mr Seah is a Director of C5 Solutions Inc, a company incorporated in the British Virgin Islands on 11 Feb 2000. Currently, the company is said to have been deregistered:

Interestingly, Mr Seah was using the address of ST Electronics as a proxy to receive correspondence with regard to his corporate affairs with C5 Solutions.He listed his address as – “c/o ST Electronics Ltd No 24 Ang Mo Kio St 65 Singapore 569061”.The abbreviation “c/o” actually means “care of”. It signifies an intermediary who is responsible for transferring mails between the postal system and the final addressee. For example when the addressee has no standard address, is under a corporate mail system, or is being manually forwarded mail sent to a more accessible location (https://en.wiktionary.org/wiki/c/o).
ICIJ said that it was releasing the new details in “public interest”, in an effort to “find out who’s behind almost 320,000 offshore companies and trusts from the Panama Papers and the offshore leaks investigations.”

In April 2016, Iceland’s prime minister, Sigmundur Davíð Gunnlaugsson, become the first major casualty of the Panama Papers leak. He resigned from his office amid mounting public outrage that his family had sheltered money offshore.

The Ministry of Finance and Monetary Authority of Singapore’s spokesman, commenting on the new Panama Papers leak implicating Singapore based entities and people said it is reviewing the papers released by ICIJ.

“If there is evidence of wrong-doing by any individual or entity in Singapore, we will not hesitate to take firm supervisory and enforcement action,” the spokesman added.

Star-studded resume of Seah Moon Ming

Mr Seah Moon Ming, who was previously the Deputy Chairman of IE Singapore, was promoted to become its Chairman in Jan 2014 (http://news.asiaone.com/news/singapore/ie-singapore-appoints-new-chairman-and-board-members). IE Singapore, a statutory board under MTI, helps to promote the overseas growth of Singapore-based enterprises and international trade.

Mr Seah took over from Mr Sunny Verghese who stepped down in Dec 2013. At the time, Mr Verghese was also the CEO of Olam International, which created news in the market scene from 2012 to 2014. Olam called the allegations “baseless rumour-mongering” and sued Block for libel later. In the end, Temasek made a bid to takeover Olam in Mar 2014. He was Senior Managing Director of Special Projects for Temasek from 1 March to 30 September 2013. Prior to joining Temasek, he was the Deputy CEO & President (Defence) of ST Engineering. He first joined the ST Group in 1994 as General Manager of CET Technologies, a subsidiary of ST Electronics. He was promoted to Managing Director of ST Electronics in July 1997 and served as ST Electronics’ President from December 1997 to August 2009.

So, this means that it was during his tenure as President of ST Electronics that he became a Director of C5 Solutions, the BVI company mentioned inside the Panama Papers. It’s not known if ST Electronics was aware of Mr Seah’s directorship in the BVI company.

Why should the leadership be faulted when they were not involved in the falsified maintenance records?

Today, my office cleaner made a mess of the floor when she spilled water on it. Should I, her employer, be held responsible for her spilling the water?
 
The "tree" was 'infected with termites' for too long a time, we living in Singapore are very blessed & fortunate people that we have the blessings. No lives were lost in the breakdown in rail services as the breakdowns has been progressively becoming more serious.

Signing off work not done, must have the culture in the work environment & ingrain into the system, this add to, no accountability of mistake made; for salaries, increments & bonuses were automatically given as long as there are upper levels you can lean on for support. "Grasslooters" oops Grass roots volunteers, Union thieves, oops! Chiefs, In other words, you can "forlumpars" or "suck on it". This is a reflection of the people who run this country...you look into the 'mirror' you will see them; no need " Harry Potter" to do that. Reflect on this.

The change is needed not only in SMRT but all companies & whatever commercial entities that are connected to the "skeleton"...the PAP. Now we see the danger we face with our lives daily commuting, placing them into people who are technically sound, proficient but utterly incompetent, add absolutely no accountability.

Not only those 'ikan bilis' get fired....the "slamon", the "whales" the heavyweights of the management all the way to the Minister in charge, must be made to be accountable for this culture of 'signing off' work...do not argue that, this is just an isolated thing...it is the culture, cultured by the decades PAP has been in power.

Yes you employ her & you have a poor judgement in giving her employment, in fairness & industrial peace, she must be called aside & talked too, issue a warning, spell out the consequences, if it occur again...issue the WC ( Walking Certificate). If this is your culture..then dunk your head in the toilet bowl after you have defecated & flush...for you are no different, from what you have flushed.

Train operator SMRT has found that the team responsible for maintaining a pump system that malfunctioned may have falsified documents by signing off on work that they had not done.


It is suspected that such falsification of maintenance records could stretch all the way back to December last year.

The lapses led to a 20-hour disruption on a section of the North-South Line last month as a train tunnel flooded after a heavy downpour.

The manager and staff responsible for the maintenance of that Bishan depot pump system have been suspended and are assisting in investigations, added SMRT in a statement yesterday evening. It said preliminary reports found that records for quarterly maintenance work as far back as December last year, and in March and June this year, were signed and submitted.

But records show the pumps were not activated on these dates, an action that would be required as part of the maintenance procedure. Nor was access to tracks issued.

The Land Transport Authority will evaluate SMRT's findings on the suspected falsification of maintenance records and make its recommendation on the penalties after full investigations.
