The US Secretary of State collapsed and hit her head after becoming dehydrated while suffering from a stomach virus in mid-December.
She was expected to return to work on Monday but a spokesman said that a test on Sunday had revealed a blood clot.
The 65-year-old was taken to New York-Presbyterian Hospital in Manhattan and is being treated with anti-coagulants.
She is expected to stay in hospital for 48 hours while doctors monitor the effect of the medication.
Philippe Reines, Mrs Clinton's spokesman, said they would look at "other issues associated with her concussion" but did not elaborate.
Mrs Clinton keeps a punishing schedule of international travel and returned from a trip to Europe in early December suffering from a stomach virus.
The illness forced her to cancel a planned trip to the Middle East where she was due to meet with the Syrian opposition and led to her calling off planned testimony before Congress on the attack on the US consulate in Benghazi.
Mrs Clinton is expected to step down from her post as America's top diplomat in January. President Barack Obama has appointed John Kerry, the veteran Massachusetts senator and 2004 presidential candidate as her replacement.
Mrs Clinton was unable to attend the brief announcement ceremony at the White House because she was recovering from the concussion.
