Getting Overstretched

Why have companies and people allowed themselves to get overstretched when the economy turns bad?

How did very prosperous companies providing valuable goods and services and ranked among the top companies in the world, suddenly find themselves unable to sustain themselves and their employees for even a couple of years? How did so many individuals find themselves in dire straits?

Is it due to over optimism of how external factors will develop? Due to over estimation of internal factors, e.g. one's own capabilities? Due to greed? Or a combination of these?

Normally these organisation are practising "hand holding", moving forward and helping their group together. But when one stand to fall, the rest will trip or lose footing too.
