WHO recommendations on pandemic (H1N1) 2009 vaccines


13 JULY 2009 | GENEVA -- On 7 July 2009, the Strategic Advisory Group of Experts (SAGE) on Immunization held an extraordinary meeting in Geneva to discuss issues and make recommendations related to vaccine for the pandemic (H1N1) 2009.

SAGE reviewed the current pandemic situation, the current status of seasonal vaccine production and potential A(H1N1) vaccine production capacity, and considered potential options for vaccine use.

The experts identified three different objectives that countries could adopt as part of their pandemic vaccination strategy:

* protect the integrity of the health-care system and the country's critical infrastructure;
* reduce morbidity and mortality; and
* reduce transmission of the pandemic virus within communities.

Countries could use a variety of vaccine deployment strategies to reach these objectives but any strategy should reflect the country’s epidemiological situation, resources and ability to access vaccine, to implement vaccination campaigns in the targeted groups, and to use other non-vaccine mitigation measures.

Although the severity of the pandemic is currently considered to be moderate with most patients experiencing uncomplicated, self-limited illness, some groups such as pregnant women and persons with asthma and other chronic conditions such as morbid obesity appear to be at increased risk for severe disease and death from infection.

Since the spread of the pandemic virus is considered unstoppable, vaccine will be needed in all countries. SAGE emphasized the importance of striving to achieve equity among countries to access vaccines developed in response to the pandemic (H1N1) 2009

The following recommendations were provided to the WHO Director-General:

* All countries should immunize their health-care workers as a first priority to protect the essential health infrastructure. As vaccines available initially will not be sufficient, a step-wise approach to vaccinate particular groups may be considered. SAGE suggested the following groups for consideration, noting that countries need to determine their order of priority based on country-specific conditions: pregnant women; those aged above 6 months with one of several chronic medical conditions; healthy young adults of 15 to 49 years of age; healthy children; healthy adults of 50 to 64 years of age; and healthy adults of 65 years of age and above.

* Since new technologies are involved in the production of some pandemic vaccines, which have not yet been extensively evaluated for their safety in certain population groups, it is very important to implement post-marketing surveillance of the highest possible quality. In addition, rapid sharing of the results of immunogenicity and post-marketing safety and effectiveness studies among the international community will be essential for allowing countries to make necessary adjustments to their vaccination policies.

* In view of the anticipated limited vaccine availability at global level and the potential need to protect against "drifted" strains of virus, SAGE recommended that promoting production and use of vaccines such as those that are formulated with oil-in-water adjuvants and live attenuated influenza vaccines was important.

* As most of the production of the seasonal vaccine for the 2009-2010 influenza season in the northern hemisphere is almost complete and is therefore unlikely to affect production of pandemic vaccine, SAGE did not consider that there was a need to recommend a "switch" from seasonal to pandemic vaccine production.

WHO Director-General Dr Margaret Chan endorsed the above recommendations on 11 July 2009, recognizing that they were well adapted to the current pandemic situation. She also noted that the recommendations will need to be changed if and when new evidence become available.

SAGE was established by the WHO Director-General in 1999 as the principal advisory group to WHO for vaccines and immunization. It comprises 15 members who serve in their personal capacity and represent a broad range of disciplines from around the world in the fields such as epidemiology, public health, vaccinology, paediatrics, internal medicine, infectious diseases, immunology, drug regulation, programme management, immunization delivery, and health-care administration.

Additional participants in the SAGE meeting included members of the ad hoc policy advisory working group on influenza A(H1N1) vaccine, chairs of the regional technical advisory groups and external experts. Observers included industry representatives and regulators who did not take part in the recommendation process in order to avoid conflicts of interest.

It’s crucial to understand that these vaccines are experimental, untested, toxic and extremely dangerous to the human immune system.

On July 13, a World Health Organization (WHO) Global Alert headlined, "WHO recommendations on pandemic (H1N1) 2009 vaccinations" suggest that universally mandated ones are coming. It stated that on July 7, the pharmaceutical industry-dominated Strategic Advisory Group of Experts (SAGE) on Immunization held an "extraordinary meeting in Geneva to discuss issues and make recommendations related to vaccine for the pandemic (H1N1) 2009."

There's no pandemic. Yet WHO said the virus "is considered unstoppable," while admitting little evidence of spread so far, most cases are mild, and many people recover unaided. Nonetheless, all countries will need vaccines and should follow these priorities as initial supplies will be limited:

-- immunize health care workers "to protect the essential health care infrastructure;" then

-- pregnant women; children over six months of age "with one of several chronic medical conditions;" healthy young adults aged 15 - 49; healthy children; healthy adults aged 50 - 64; and finally healthy adults aged 65 or older.

WTO suggested the risks in stating "new technologies are involved in the production of some pandemic vaccines, which have not yet been extensively evaluated for their safety in certain population groups..." As a result, "post-marketing surveillance" and "post-marketing safety and effectiveness studies" are essential so that countries can adjust their vaccination policies.

WHO "recommendations" are binding on all 194 member countries in case a pandemic emergency is declared under the 2005 International Health Regulations Act and April 2009 WHO pandemic plan.

It's crucial to understand that these vaccines are experimental, untested, toxic and extremely dangerous to the human immune system. They contain squalene-based adjuvants that cause a host of annoying to life-threatening autoimmune diseases. They must be avoided, even if mandated. It's also known that vaccines don't protect against diseases they're designed to prevent and often cause them. They should be banned but proliferate anyway because they're so profitable, and if globally mandated to the greatest extent ever.

Get ready because that's precisely what's coming - universal orders to risk toxic vaccine hazards. In the coming weeks, the dominant media globally will get into high gear fear-mongering mode to convince people voluntarily to submit to jeopardizing their health and well-being. It's essential to refuse and be safe and international law absolutely protects us.

It's emerged that virulent H5N1 bird flu was sent out by accident from an Austrian lab last year and given to ferrets in the Czech Republic before anyone realised. As well as the risk of it escaping into the wild, the H5N1 got mixed with a human strain, which might have spawned a hybrid that could unleash a pandemic.

Last December, the Austrian branch of US vaccine company Baxter sent a batch of ordinary human H3N2 flu, altered so it couldn't replicate, to Avir Green Hills Biotechnology, also in Austria. In February, a lab in the Czech Republic working for Avir alerted Baxter that, unexpectedly, ferrets inoculated with the sample had died. It turned out the sample contained live H5N1, which Baxter uses to make vaccine. The two seem to have been mixed in error.

Markus Reinhard of Baxter says no one was infected because the H3N2 was handled at a high level of containment. But Ab Osterhaus of Erasmus University in the Netherlands says: "We need to go to great lengths to make sure this kind of thing doesn't happen."

Accidental release of a mixture of live H5N1 and H3N2 viruses could have resulted in dire consequences. While H5N1 doesn't easily infect people, H3N2 viruses do. If someone exposed to a mixture of the two had been simultaneously infected with both strains, he or she could have served as an incubator for a hybrid virus able to transmit easily to and among people.

Doctors to issue 'fast track' death certificates for swine flu

Doctors are to be allowed to issue "fast-track" death certificates under Government plans to help the health system cope with the workload at the height of the swine flu pandemic.

Sir Liam Donaldson, Chief Medical Officer for England, confirmed the planned move in an interview on BBC's Newsnight.

"We want to try and reduce as much as possible the burden of work on doctors and we're considering all sorts of things which wil help with that," he said. "It's one of a number of things that we hope at the height of the pandemic - which we may see in the autumn and winter - will reduce the burden of paperwork for doctors."

But Sir Liam, the Government's principal advisor on public health issues, disputed a report from researchers at Imperial Collge London suggesting that 0.5 per cent - or one in 200 - of those who are ill enough to seek help for swine flu will go on to die as a result of it.

That projection means that there could be as many as 500 people a day dying of the virus by the end of August, by which time, according to the Health Secretary Andy Burnham, the daily infection rate could have hit 100,000.

"The range that we're working with is lower than the Imperial College predictions," Sir Liam said. "But the point is that nobody can be absolutely sure what level of mortality we'll actually see. It's very early days."

Separately, Sir Liam disputed comments from Dr Margaret Chan, director-general of the World Health Organisation, who cast doubt on minister's claims that the UK would start receiving its first stocks of swine flu vaccine this summer.

Dr Chan told the Guardian that a vaccine could be available as early as August. "But having a vaccine available is not the same as having a vaccine that has been proven safe," she added. "Clinical trial data will not be available for another two or three months."

The WHO chief's comments came as Australia and Japan reported a surge in cases of the H1N1 virus, and Argentina dramatically upped its death toll from 94 to 137 in just three days. So far 16 people are known to have died in the UK after contracting the virus.

Responding to Dr Chan's comments, Sir Liam said: “She may be commenting from a global perspective, but as far as the UK is concerned, we are still expecting to get 60 million doses of vaccine by the end of the calendar year and for the first supplies to arrive in the early autumn.

“There is some uncertainty because vaccine manufacture involves a complex process of testing biological products, so it can sometimes go wrong and cause delays. We then have to license it properly and that may take a little time, but we still will be one of the first countries in the world to get vaccine.”

Asked again about the likely mortality rate, Sir Liam said that he would issuing new planning assumption to the NHS later this week. “They need to know broadly what numbers of patients might be hospitalised, what sort of pressure will be put on the NHS," he said. "Statistical modelling is very valuable, but we do need to treat it with caution early on.”

The number of confirmed cases of swine flu has topped over a hundred thousand, with the World Health Organization calling the pandemic 'unstoppable', and suggesting mass vaccination.

Investigative journalist and RT contributor, Wayne Madsen from Washington, says that the world is actually fighting a man-made tragedy. He says the University of Wisconsin-Madison, was involved in the development of the swine flu virus.

“The 1918 Spanish flu was extracted from the corpse of a dead Aleut woman, who died of the disease in a small village of Alaska, and it was combined with other forms of flu to create this particular H1H1 that was declared a pandemic by the World Health Organization.”

Madsen points out that, “A company called FluGen, which is associated with this research at the University of Wisconsin-Madison, is now developing a vaccine. I think what we see with Baxter International, FluGen and other companies developing a vaccine when some of these companies were involved in the research of this particular form of flu, so people are a little bit skeptical about the profit motivation here.”:eek:

Cherie 'has suspected swine flu'

Cherie Booth has pulled out of receiving an honorary degree at Liverpool Hope University because she has suspected swine flu.

The former PM Tony Blair's wife was due to be honoured with a Doctor of Laws for her work in human rights and her support for the university.

It is unclear if her husband or any of their four children have the virus.

The news came as GPs warned the number of people fearing they had swine flu had jumped 50% to 40,000 last week.

Mrs Blair, who has a high flying legal career, was brought up not far from the university. In her memoirs published in May 2008 she described herself as just "a girl from Liverpool who found herself at No 10 Downing Street".

First off, unlike SARS, its not really that deadly, 2ndly, doing that might force the virus to evolve into an even more dangerous and deadly strain. The best thing to do right now is try to stay healthy and practice good hygene.

ATLANTA — The last time the government embarked on a major vaccine campaign against a new swine flu, thousands filed claims contending they suffered side effects from the shots. This time, the government has already taken steps to head that off.

Vaccine makers and federal officials will be immune from lawsuits that result from any new swine flu vaccine, under a document signed by Secretary of Health and Human Services Kathleen Sebelius, government health officials said Friday.

Since the 1980s, the government has protected vaccine makers against lawsuits over the use of childhood vaccines. Instead, a federal court handles claims and decides who will be paid from a special fund.

The document signed by Sebelius last month grants immunity to those making a swine flu vaccine, under the provisions of a 2006 law for public health emergencies. It allows for a compensation fund, if needed.

The government takes such steps to encourage drug companies to make vaccines, and it's worked. Federal officials have contracted with five manufacturers to make a swine flu vaccine. First identified in April, swine flu has so far caused about 263 deaths, according to numbers released by the Centers for Disease Control and Prevention on Friday.

The CDC said more than 40,000 Americans have had confirmed or probable cases, but those are people who sought health care. It's likely that more than 1 million Americans have been sickened by the flu, many with mild cases.

The virus hits younger people harder that seasonal flu, but so far hasn't been much more deadly than the strains seen every fall and winter. But health officials believe the virus could mutate to a more dangerous form, or at least contribute to a potentially heavier flu season than usual.

"We do expect there to be an increase in influenza this fall," with a bump in cases perhaps beginning earlier than normal, said Dr. Anne Schuchat, director of the CDC's National Center for Immunization and Respiratory Diseases.

On Friday, the Food and Drug Administration approved the regular winter flu vaccine, a final step before shipments to clinics and other vaccination sites could begin.

The last time the government faced a new swine flu virus was in 1976. Cases of swine flu in soldiers at Fort Dix, N.J., including one death, made health officials worried they might be facing a deadly pandemic like the one that killed millions around the world in 1918 and 1919.

Federal officials vaccinated 40 million Americans during a national campaign. A pandemic never materialized, but thousands who got the shots filed injury claims, saying they suffered a paralyzing condition called Guillain-Barre Syndrome or other side effects.

"The government paid out quite a bit of money," said Stephen Sugarman, a law professor who specializes in product liability at the University of California at Berkeley.

Vaccines aren't as profitable as other drugs for manufacturers, and without protection against lawsuits "they're saying, 'Do we need this?'" Sugarman said.

The move to protect makers of a swine flu didn't go over well with Paul Pennock, a prominent New York plaintiffs attorney on medical liability cases. The government will likely call on millions of Americans to get the vaccinations to prevent the disease from spreading, he noted.

"If you're going to ask people to do this for the common good, then let's make sure for the common good that these people will be taken care of if something goes wrong," Pennock said.
